Case entry is designed with radio buttons to maximize data input efficiency for the busy electrophysiologist. Input fields include patient medical record number, hospital, attending physician, referring physician, and a note section to allow the user to free text specific information about the procedure. This database is searchable and can be exported to a password protected Excel spreadsheet to your secure e-mail or desktop.
