• Police Certificate for children of unknown origin, and

• Certificate from social institutions for the birth of vulnerable population children.

How to make a birth certificate is to complete the following documents:

• Birth Certificate from the Kelurahan

• Original and photocopy of Birth Certificate from the doctor / midwife / birth helper / Shipmaster or Airplane Pilot by showing the original

• Marriage Certificate / Parent's Marriage Certificate

• Photocopy of parent's KK and KTP

• Name and identity of witnesses reporting birth

• Approval of the Head of Service / Department of Service in the event that the report exceeds 60 days from the date of birth.
